2009 Golf Digest U.S. Open Challenge
Eligibility : 2009 Golf Digest U.S. Open Challenge (the “Promotion”) is open only to legal residents of the fifty (50) United States (including District of Columbia) who are at least eighteen (18) years old at the time of entry.
Promotion Period : The 2009 Golf Digest U.S. Open Challenge begins on November 11, 2008 at 12:01 a.m. Eastern Time (“ET”) and ends on May 1, 2009 at 11:59 p.m. ET (the “Promotion Period”).
Limit : You may enter the Sweepstakes one (1) time per day and submit one (1) Essay and one (1) Optional Video total during the Upload Phase.
For each referral you make with a valid email address, you will automatically receive one (1) additional entry into the Sweepstakes, for a total of up to five (5) additional entries per day during the Upload Phase through the Tell A Friend Program.
Prize (s):
Contest Prize: ONE (1) PRIZE: Trip for two (2) to the 2009 U.S. Open Challenge and the opportunity for the winner only to play golf with three (3) celebrities. Actual date of round will occur between June 7, 2009 and June 13, 2009. Trip package includes roundtrip, coach-class air transportation for two (2) from a major airport near winner’s home (determined by Sponsor in its sole discretion) to New York, NY, three (3) days, two (2) nights accommodations at a hotel determined by Sponsor in its sole discretion (single room, double occupancy); and the opportunity for winner only to play golf at Bethpage State Park, site of the 2009 U.S. Open with three (3) celebrities and be filmed by NBC Sports (specific celebrities and all details will be determined by Sponsor in its sole discretion). Approximate Retail Value (“ARV”): $2,280.
Sweepstakes Prize: ONE (1) GRAND PRIZE: Trip for two (2) to the 2009 U.S. Open at Bethpage State Park in Farmingdale, NY. Trip package includes roundtrip, coach-class air transportation for two (2) from a major airport near winner’s home (determined by Sponsor in its sole discretion) to New York, NY; five (5) days, four (4) nights accommodations at a hotel determined by Sponsor in its sole discretion (single room, double occupancy); and two (2) tournament passes (Thursday to Sunday) passes to the 2009 U.S. Open at Bethpage State Park. ARV: $3,720.

I was unable to leave my essay and video with my US Open entry today. I was unable to find a place on the entry form for either. Please advise as to the proper procedure to accomplish the requirements of the submittal.
